Each certificate is personalized with care and created completely free of charge.

A Certificate of Life is a personalized memorial certificate created to honor your baby's life. While it is not an official birth certificate, it is a beautiful keepsake that celebrates your baby's place in your family and your heart.

Every certificate is thoughtfully designed with compassion and personalized using the information you provide.

To create your personalized certificate, I will need:

  • Parent(s) name
  • Baby's name
  • Birth date
  • Passing date (if applicable)
  • Weight 
  • Length 
  • Hospital 
  • Up to two favorite photos 
  • Your email address
